It's from Latin origin. And, if I remember correctly, it means out of.

So, in the context of ex - girlfriend etc. You could say that the girl is out of the relationship. At least, that's how I always understood it.

From Dictionary.com, ex- (http://dictionary.reference.com/search?q=ex-&x=37&y=16)
ex-1
a prefix meaning “out of,” “from,” and hence “utterly,” “thoroughly,” and sometimes imparting a privative or negative force or indicating a former title, status, etc.; freely used as an English formative: exstipulate; exterritorial; ex-president (former president); ex-member; ex-wife.
Hannes, you understand correctly! :)

It offers different meanings in writing and listening

Doesn't EX sound like a single X ?
Independence or In dependence just sounds the same ?

I like to shorten my writing of EXERCISE to just EX
