I'm new to mysql and I'm not sure how to lay out this table.

I have the html and php all written out and working, now im adding the database end. My goals are to create a searchable db referenced by usernames (but that will probably be changed to some sort of key). This db will be accessed by users a number of times, each time data they enter into a textarea should be added to the database and the corresponding date/time of the entry should be added as well. Can anyone tell me how to lay out my table so that this can be done in the most painless way?

BTW I'm also sketchy on how to created records (rows columns whatever) in php, anyone got some resources or examples to add? thanks.
